Diagnose probs

I'm getting two conditions when I diagnose.
A ruddy-cheeked, curly-haired lass
She appears to be slightly the worse for wear.
She appears to be in excellent condition.
A rotund, ruddy-faced man
He appears to be slightly the worse for wear.
He appears to be in excellent condition.
A flame-haired, flush-cheeked man
He appears to be slightly the worse for wear.
He appears to be in excellent condition.

Has anyone been able to replicate this lately? I can't seem to make it work for me.

Diag has seemed to work fine for me. When at 5 Stars you get that responce, at 4 you get injured and so forth.

Doesn't that happen when the PC(or npc) has suffered from bleed damages?
